I understand and agree that any offer of employment from Castillo Prestress is contingent upon the successful completion of background checks conducted after a successful interview, as permitted by applicable law.
I understand that additional screenings, including fingerprint-based background checks or other verifications, may be required depending on the position, job duties, work location, or client or site-specific requirements.
I understand that my employment may be terminated or an offer of employment may be withdrawn if any information provided by me in this affidavit or during the application process is found to be false, incomplete, or misrepresented.
By signing this document, I authorize Castillo Prestress to conduct background checks, including the use of my Social Security number, for employment-related purposes.
I understand that these background checks may include, but are not limited to, a Social Security number trace, nationwide criminal database search, sex offender registry search, and applicable government or compliance watch lists.
In connection with any request for or provision of such information, I expressly release and waive any claims against Castillo Prestress, its owners, officers, employees, agents, representatives, and any third-party providers of background check services, including but not limited to claims for defamation, invasion of privacy, emotional distress, or interference with contractual relations, except as prohibited by law.
I understand that all offers of employment and terms of employment are conditional until the required background investigation is completed.
I have read and understand this authorization and release, and I voluntarily agree to the terms set forth herein.
I understand by signing this document, I am authorizing Castillo Prestress to run a social security background check using my social security number. I understand the background checks will gather information from the Domestic Watch List, Nationwide Criminal Database, Sex Offender registry and a Social Security Number Trace. In connection with any request for or provision of such information, I expressly waive any claims, including without limitation defamation, emotional distress, invasion of privacy or interference with contractual relations that I might otherwise have against the District, its agents and officials or any provider of such information.
